
Several barriers continue to limit patient access to traditional facility-based programs. Transportation challenges, work and caregiver responsibilities, financial limitations, and geographic “rehab deserts” all contribute to underutilization. Addressing these challenges is more than an operational concern — it is a public health priority.
To help address this gap, LSI has developed a new resource: Home-Based Cardiac Rehab: A Hybrid Model for Expanding Access. This document explores how combining in-person care with remote and virtual solutions can create more flexible, scalable cardiac rehab programs. Hybrid models allow providers to extend care beyond the clinic, improving accessibility while maintaining clinical oversight and quality outcomes.
